What do I do if I run out of space in the Clinical Treatment / Procedures section?

Start a second ACR. Strike through the front page of the second ACR with a single line and initial it, then make sure the Call # and page numbers are recorded accurately on both forms.

Do I have to enter the call number on the second page?

Yes — the call number must be recorded on both pages of the ACR.

Can I enter "Transfer" or "MVA (MVC)" in the Chief Complaint area?

No. The Chief Complaint should reflect the patient's expressed issue — in the patient's own words whenever possible. A mechanism of injury (like an MVC) isn't a complaint, so it doesn't belong in this section.

How do I know which code goes in Primary Problem vs. Secondary Problem?

Choose the codes that best represent the patient's primary issue. They're listed on the back of the ACR, and the complete list of ministry problem codes is available on ontario.ca.
